Letter of Intent and Background Check
For the safety of our players, and according to new ASA requirements, all
prospective managers and coaches must submit two forms:
If you are planning to be a manager or (assistant) coach and
did not fill in these forms at the registration, please do so as
soon as possible and mail them to
SMGSA, P.O. Box 1055, Sierra Madre, CA 91025-4055
Please note that you must attach a copy of a government-issued photo ID
(driver's license or state ID), either in the space at the bottom of
the background check form, or on a separate sheet of paper.
The background check is mandatory: you will not be allowed to coach
unless we have this form on file.
